60Y FEMALE WITH RIGHT HEMIPARESIS

I have been given this case to solve in an attempt to understand the topic of  " patient clinical data analysis" to develop my competency in reading and comprehending clinical data including history, clinical findings, investigations and come up with diagnosis and treatment plan.




A 60/F K/c/o HTN since 1.5 year & left hemiparesis (right temporo parietal infarct) 1yr back on Rx (antiplatelets) came to casualty at 6:30pm on 16/6/2021 with c/o •Slurring of speech since 1 week,
•Generalized weakness since 1 week,
•Altered sensorium since 15/6/2021 night.

HISTORY OF PRESENTING ILLNESS:

Patient was apparently asymptomatic one and half  year back. Then she was diagnoesd with hypertension for which she was on medication. 1 year back she had slurring of speech and weakness on left side both the limbs for which she got admitted and was treated and on continuous antiplatelet medication. She was fine doing her regular work by herself with little help until 7/6/2021. 
From 7/6/2021 she again had symptoms like •Slurred speech and generalised weakness •Altered sensorium since 15/6/2021 for which she was brought to the causality on 16/6/2021.

No h/o trauma/ head ache / giddiness / LOC / involuntary movements
No h/o fever / vomiting / loose stools
No other complaints



O/E:
pallor: absent
Icterus: absent
Cyanosis: absent
Clubbing: absent
Edema: absent
Lymphadenopathy: absent

VITALS : ( on presentation)
GCS : E2V1M4
Pulse Rate : 150bpm
Blood Pressure : 140/90 mmhg
Sp02: 98% on RA
Temperature: Afebrile

Murmurs: not heard

No organomegaly 
Bowel sounds heard

Provisional Diagnosis:-
RIGHT HEMIPARESIS ( Acute infarct in left lentiform nucleus and head of caudate extending into corona radiata , multiple acute lacunar infarcts in left occipital  and peritrigonal zone) .
K/C/O Left hemiparesis (1y back) 
K/C/O HTN (1.5y back).
